INCOMPLETE GRADE REQUEST FORM

Note: An INCOMPLETE grade should be requested if there are specific, acceptable reasons why an assignment or test cannot be completed during the academic term. After the student and professor/instructor have agreed upon the need for an INCOMPLETE, the professor/instructor must complete and submit this form to the Office of Academic Affairs.
College Catalog: Incomplete means that a student has done satisfactory work in a course but has failed to complete a portion of the course requirements because of documented, extenuating circumstances. Note: an email will be sent to the student with all of the following information as a confirmation of the incomplete request.

Please state the specific assignments/tasks the student must perform to complete the course.
Bacone policy requires that all incompletes be satisfied by the middle of the next academic term.
A final grade MUST be indicated.
